i killed patchwerk and didnt get the ok for killing him

i then abandoned the quest then rejoined it and patchwerk did not rez

i did the same as before then reset the instance still nothing

i thine cleaned out the cache still the same

i then did all 3 things got nothing

can some one help me with this issue
